What You Need to Know Before You Arrive
Welcome to Grand Dental – South Elgin! Our dental office is designed to provide patients of all ages with a safe, comfortable, and welcoming environment. Dr. Siewert and our team members are equipped to deliver exceptional care using only the best technology in a state-of-the-art facility. If you are new to the area and/or our office, we invite you to review the information below so that you are prepared for your initial visit. Your First Visit
Your arrival at our South Elgin dental office will include a warm greeting from a member of our front desk staff before being escorted to a treatment room. After capturing digital images of your mouth, we will begin to perform a dental checkup before analyzing our findings. A thorough cleaning of your teeth and gums will also be included before Dr. Siewert arrives to go over your information and discuss any proposed treatment plans. After learning more about you and how we can help, you will meet with a member of our team to discuss available payment options.
New Dental Patient Forms
Instead of waiting until you get to our dental office to fill out your new patient forms, you can easily access them online. By filling out your information beforehand, you reduce the amount of time you’ll spend in our waiting room. Should you have any difficulties accessing the forms or filling them out, let us know by calling our office.

Dentistry doesn’t have to be a cumbersome investment. Instead, Dr. Siewert is pleased to be in-network with multiple dental insurance companies that make it possible to pay less for various restorative and preventive treatments.

Grand Advantage Plan
Military Discount
Veterans and their families enjoy the benefits of the Grand Advantage Plan plan on us!
Veterans of the United States Military that do not have any other source of dental insurance or dental discount plans may participate in the Grand Advantage Plan (GAP) at no cost to join. GAP is a membership plan that was created by Grand Dental to provide affordable dental care for individuals who may require support to access the oral healthcare they need and deserve. GAP provides a 20% discount on all services offered at any Grand Dental Group location for a $49 annual fee to join. Veterans, who fit the above criteria, and their immediate family members are invited to join our GAP program for free. Grand Dental Group will pay the annual membership fees. Rules and restrictions apply.

- Veteran may be asked to provide proof of veteran status to activate and/or maintain GAP membership
- GAP is not available with any other offer, coupon, discount, discount plan, or insurance. If the Veteran has other insurance, GAP will only apply to non-covered services, at our usual and customary fee, less 20%.
- All fees for service must be paid at time of service
- Program not available for VA Referral patients
- Veterans are eligible for Free Teeth Whitening for Life
- Veterans may participate in our referral bonus program without restriction
Financing Available
When the cost of treatment is not covered by your dental insurance, or you cannot afford to pay the full amount upfront, you can apply for easy financing through CareCredit or Alphaeon. As third-party financiers, they offer flexible payment plans that are low-to-no-interest so that you can better budget for your upcoming treatment.
